About CNYHHN
Central New York Health Home Network (CNYHHN) provides Health Home Care Management Services that is a collection of six core services to assist high risk and high need individuals with complex combinations of chronic conditions and/or severe mental illness with Medicaid so that they can better utilize health care, social services, and community and natural supports.
Providing care management services since July of 2013, our trained workforce of care managers ensures that everyone involved in a person’s care – from doctors to community service members is working together to help create the best possible lives for people.
We continue to grow our services and expand on collaboration within the community. CNYHHN also consists of three core programs that serve families and children in our community. These programs are Connected Community Schools (CCS), Family Advocate Connections Team (FACT) and Restorative Integrated Youth Services (RIYS).
why CNYHHN?
For Clients: CNYHHN serves individuals with SMI, serious mental illness and chronic conditions. We work in collaboration with local providers and clinics to provide a Person-Centered Plan of Care so that our clients can have a better quality of life. We have established strong relationships with the community around us and can link our clients to the services they need.
For Partners: As of March 1, 2017 we have been designated by the New York State Department of Health as a Health Home Serving Children and can now expand even further out into the community to serve an additional population, children.
Care Management Services Referral
We are accepting referrals from the community for enrollment of eligible individuals into Health Home Services. You can make a referral for yourself or a family member. Individuals must meet all eligibility requirements to be considered for enrollment. Please download the appropriate referral form, provide the necessary information, and click submit.
